MEMBERSHIP APPLICATION

Before starting the process of becoming a member of the 4Seasons Collingwood PROBUS Club (The Club), please familiarize yourself with our history, our constitution, and the types of activities and events we enjoy. If you see this as a fit for you, then you're most welcome to become one of us!

The Club only offers individual memberships. Each application is for one person only. If you are applying as a couple, you must each complete an independent application.

Please print, read, sign and upload our waiver in the Waiver field of the following on-line application.

The waiver can be digitally completed by following these instructions.
